Clinical Integration
Clinical integration is the alignment of the HPHO medical community around common clinical goals and strategies designed to promote high quality, cost-effective healthcare. It is also the ability to measure and compare outcomes for the population of patients we manage, and to improve the quality and efficiency of that care. Current HPHO clinical integration activities focus on expansion of disease registry programs using the Medventive registry engine, outpatient care management and pay-for-performance initiatives.
The HPHO will follow these guiding principles when implementing the clinical integration program:

Initiatives must improve quality and/or decrease costs

Focus on process AND outcomes

Need strong and fair rewards and/or sanctions policies

Achieve broadest possible participation of physician members

Enhance communication between providers, especially PCPs and specialists
The HPHO Clinical Integration Program currently includes, but is not limited to:

Measuring and improving quality of care provided to patients with chronic conditions in outpatient settings using a web-based, real-time registry

Case management of patients identified through the disease registry who may need coaching with self-management so they can reach and maintain optimal control of their chronic condition(s)

Measuring surgical site infection rates as identified in the post-discharge visit to the surgeon

Case management of patients for thirty days post-discharge from Hartford Hospital to prevent avoidable readmissions
