HPHO Case Management Program

The HPHO Case Management Program is focused on two populations at this time:

• Pilot program between Hartford Hospital and an insurance carrier to reduce avoidable hospital readmissions. The case manager facilitates communication between hospital, sub-acute, home care and outpatient providers, and acts as a patient advocate, health coach and care coordinator for patients insured by this payor, beginning upon discharge from Hartford Hospital.  Case management has improved coordination of care for patients and families, and resulted in lower readmission rates for this payor's population.
• Chronic condition management for patients identified through the HPHO chronic disease registries for diabetes, coronary artery disease and congestive heart failure, as well as Hartford Hospital employees with chronic conditions.  Case managers support patients by adhering to evidence-based care guidelines and collaborate with the patient’s treating physician to help patients and families do a better job of caring for themselves.
• Congestive heart failure (CHF) readmission reduction project at Hartford Hospital.  Case managers are working with patients hospitalized at Hartford Hospital to coordinate care in post-acute settings to improve the quality of patients’ care and reduce avoidable readmissions.

HPHO Case Management Staff:

• Lucille Taylor, RN, MBA has extensive patient care experience, including having served as a Care Coordinator at Harford Hospital, Nursing Director at Interim Healthcare (a home nursing agency), Nursing Manager/Director at the Hospital of Central Connecticut, the Hospital for Special Care and Hartford Hospital, Case Manager at St. Francis Hospital, Bridgeport Hospital and Case Manager at ConnectiCare
• Linda Conroy, RN, BSN has broad experience as a health care provider, including Care Coordination at Hartford Hospital and in the Hartford Hospital outpatient clinics, Clinical Research on chronic disease management at HH, Quality Management at CIGNA Healthcare, Primary Care Nursing at St. Francis Hospital and home nursing, including hospice, at the Farmington Valley Visiting Nurse Association
• Both HPHO case managers are committed patient advocates, with experience using motivational interviewing to help patients participate in their own care
